Southern New England Practice Transformation Network to share recruitment success at 2016 CMS Quality Conference

The successful recruitment strategies of the Southern New England Practice Transformation Network (SNE-PTN), a collaborative led by UMass Medical School and UConn Health, were selected by the Centers for Medicare and Medicaid Services (CMS) to present in a poster at the 2016 CMS Quality Conference at the Baltimore Marriott Waterfront Hotel in Maryland Dec. 13-16.

The poster will outline the strategies used by SNE-PTN to enroll more than 5,000 clinicians to date from a variety of specialties, achieving 186 percent of the year one enrollment target. 

SNE-PTN is one of 29 practice transformation networks across the country chosen by CMS to be part of its Transforming Clinical Practices Initiative (TCPI). This practice transformation program aims to bring together specialty and primary care clinicians to work individually and collaboratively to transform care delivery under rapidly evolving payment models.

SNE-PTN’s network partners include Baycare Health Partners, eHealthConnecticut, Healthcentric Advisors, Massachusetts Health Policy Commission, Qualidigm, Qualis Health and SMC partners, LLC.

U.S. Health and Human Services Secretary Sylvia M. Burwell announced in September 2015 that SNE-PTN would receive $29.2 million to provide technical assistance support to clinicians.
